Chronic Management Plan (CMP/ EPC) Program

At Refine Health Group, we do what we can to make life-changing remedies as accessible as possible. In this case, it’s through a Chronic Management Plan Program (CMP). These are a means for general practitioners (GPs) to better manage health care, including multidisciplinary care from a GP and Allied Health services to patients with chronic or terminal medical conditions – such as a medical condition that has been, or will be present for a minimum of 6 months.

This makes patients eligible for physio Medicare rebates for only certain Allied Health services through the GPs referral.

How to Get a CMP Referral

If you’re wanting to know how you can get a CMP program referral for yourself, it will be provided by your GP – but only if you are deemed eligible for this service following a clinical judgement from the GP, where they will consider your medical condition/s and healthcare needs. Keep in mind that a CMP program only allows for a maximum of 5 Allied Health service rebates a year.
